About the Role
The Care Navigator provides direct support to individuals impacted by neurodegenerative conditions. This role manages patient relationships, enrollment, documentation, and overall care experience within the Synapticure program, while also helping shape workflows and processes for the Care Coordination team in collaboration with peers and leadership. Ideal candidates are passionate about elevating patient voices, thrive in direct support roles—especially with vulnerable populations—and bring thoughtfulness, organization, curiosity, compassion, and empathy. They are driven to make a meaningful impact on patients’ lives and eager to contribute to a program that is rapidly growing and evolving to better serve the neurodegenerative community.
Job Duties – What You’ll Do
• Build and maintain supportive relationships with patients living with neurodegenerative diseases and their caregivers, serving as their main point of contact.
• Follow HIPAA standards, carry out patient care plans, and offer ongoing support to coordinate connections with neurology and genetic experts and various referral resources.
• Provide basic health education, including information on diagnoses, treatments, care options, and research opportunities.
• Ensure high-quality interactions by maintaining strict standards of privacy and confidentiality.
• Develop and follow standardized procedures, including operations, quality assurance, and documentation, to improve efficiency and effectiveness.
• Foster positive collaboration with the multidisciplinary Care Coordination team and the broader Synapticure staff to ensure a seamless patient experience. Represent Synapticure effectively when working with internal and external partners.
• Build and sustain strong relationships with medical and community partners.
• Stay informed about research developments and advances in neurodegenerative care and speak confidently about the program and company.
Requirements – What We Look For
• Bachelor’s degree in a related field
• At least 2 years of direct case or care management experience in a healthcare setting
• Comfort using technology for remote support, including phone, text, and virtual visit platforms
• Excellent communication, organization, and interpersonal skills for effective teamwork
• Ability to use scheduling platforms for accurate appointment coordination
• Ability to use electronic health record or care facilitation systems for accurate documentation
• Skilled in gathering and documenting clinical and demographic information promptly
• Strong problem-solving abilities and good judgment on when to collaborate
• Creative thinking to address barriers within the healthcare system
• Growth mindset and willingness to go beyond traditional responsibilities
• Adaptability in a fast-changing environment with shifting priorities
• Ability to build cooperative relationships with patients, colleagues, and healthcare and community partners

Travel Requirements
This is a fully remote role with provided home-office technology. Occasional travel to Chicago, IL or other locations may be required.
Salary and Benefits
This is a full-time, exempt position with a competitive salary and comprehensive benefits, including health insurance. Salary is commensurate with experience.
