Your role and impact 98point6 is looking for a Mental Health Technician with a track record of success in working on interdisciplinary teams to help support behavioral health patients along the care journey and enhance the care experience. You’ll be responsible for facilitating patient engagement, assisting with identifying outside resources and coordinating follow-up care. These opportunities are essential to our company’s long-term mission and to the patient care experience. Responsibilities
Support the mental health care of 98point6 patients by coordinating care with 98point6 and external providers
Understand diagnostic screenings used to determine care pathways for patients
Create and support safety plans for patients at risk for suicide or other harm
Collaborate with the Behavioral Health Director and Managers in managing patient safety and communicating scope of practice through care agreements and other engagement strategies as needed
Provide patient resources and education around common mental health disorders and the available treatment options
Perform patient follow-up to identify and support patient well-being, safety plan compliance and care needs
Support patient understanding of and adherence to medical care plans
Facilitate referrals and additional care needs for patients between 98point6 visits and external providers as well as to community and public health resources
Collaborate with cross-functional teams and providers to deliver safe, efficient and equitable patient care
Document patient progress towards treatment recommendations, track clinical outcomes and identify patients requiring further engagement
Support patients with creative solutions when barriers arise in their care plans, while complying with applicable laws and regulations
Qualifications
Mental Health Technician certification required
3+ years of experience working in a behavioral health setting
Experience in care coordination and case management preferred
Experience with telehealth preferred
Exposure to an outpatient behavioral health setting preferred
Crisis management experience preferred
Skills in Motivational Interviewing or similar behavior change modality preferred
Exceptional written and verbal communication
Excellent organizational skills and the ability to collect, analyze and synthesize data
Comfortable working with technology
High emotional intelligence and empathy
Ability to be flexible and pivot easily when needed
