Position Summary: Spring Health is seeking a Care Navigator/Support Coordinator to assist members through their mental health care journey and connection to care while ensuring a high-quality member experience. The role reports to the Manager of the Global Team in Clinical Operations and requires a deep understanding of patient and clinician workflows, strong responsiveness, resourcefulness, empathy, and professionalism.
Working Hours: Monday to Friday, 9:00 AM to 5:00 PM EST
Responsibilities:
- Use active listening and written communication to support patients via phone, chat, and email in the language(s) spoken fluently.
- Manage care coordination needs, including platform sign-up support, troubleshooting technical issues, gathering intake information, scheduling and canceling appointments, and explaining available services.
- Conduct clinical needs assessments and develop plans of action for each member, referring them to the appropriate level and modality of care.
- Assess and synthesize information using clinical judgment to support member crises such as suicide risk through in-the-moment support, triage, safety planning, and follow-up.
- Provide consultation, psycho-education, empathy, and motivational assistance for mental health, substance use, and family/relationship concerns.
- Engage in CIR intake and/or coaching services for global members and customers.
- Identify and resolve issues that create barriers or disruptions to care, technical problems, and related concerns submitted by members and providers.
- Research and document crisis resources for members in different countries.
- Review workflows, training, and policies written and spoken in English.
- Summarize all patient interactions in English within Spring Health’s online documentation systems.
- Coordinate with internal teams to identify, report, and resolve issues and ensure a smooth member experience.
- Participate in staff meetings, case consultations, training, and internal projects as assigned.
- Follow Spring Health policies and maintain confidentiality, compliance, and ethical standards.
Requirements:
- Master’s degree in a behavioral health field and licensure or certification as a counselor, social worker, psychotherapist, or psychologist.
- Appropriate credentials in the candidate’s U. S. state or country of residence.
- Minimum of 2+ years of direct clinical care experience after licensure.
- Excellent written and verbal English and Italian communication skills.
- Comfort interacting with members worldwide.
- Ability to use multiple computer platforms simultaneously and learn new systems.
- Strong motivation to provide high-touch service and outstanding customer support.
- Ability to thrive in a fast-paced and ambiguous environment.
KPIs: Connection to care, productivity, and member satisfaction.
Compensation: Target base range of $33.33 to $42.31 per hour, plus a competitive total rewards package including benefits.
