Job Summary:
We are looking for a dedicated and compassionate Wound Care Nurse and Telehealth Coordinator to support our skilled nursing facility.
The Wound Care Nurse and Telehealth Coordinator serves as the connection between healthcare providers and patients. In addition, the role assists with wound dressing changes under direct supervision, using real-time imaging technology.

Responsibilities:
– Provide on-site, hands-on support to help conduct weekly virtual wound rounds using technology, as directed by wound care specialists, nurses, or other healthcare providers.
– Capture wound photos using the required telehealth equipment and ensure accurate documentation for clinical review.
– Assist patients with telehealth platform use, troubleshoot technical issues, and help maintain reliable video connections for appointments.
– Coordinate telehealth scheduling for patients and provide education for virtual visits to patients and staff as needed.
– Perform all wound care duties in line with evidence-based practice and facility policies, including dressing changes and participation in wound rounds.
– Follow strict patient confidentiality and privacy requirements, including HIPAA, for all telehealth communications and records.
– Submit orders for wound care products.
Qualifications:
– Graduation from an accredited nursing program is required. Current CPR certification is required. Minimum one (1) year of wound care experience is required; two (2) years is preferred. Wound care certification is preferred. Must have a current, unencumbered, active RN or LPN license in the state of practice.
– Strong communication skills, including the ability to clearly explain medical information to physicians, staff, patients, and families.
– Compassion, patience, and a genuine commitment to delivering high-quality care at the bedside and through telehealth.
– Commitment to maintaining patient confidentiality, privacy, and data security in accordance with healthcare regulations (e.g., HIPAA).
– Ability to multitask and adapt to changing telehealth procedures, job requirements, and patient needs.
– Experience in SNF, LTC, or Assisted Living is preferred.
– Ability to travel to assigned facilities by personal vehicle with a valid driver’s license; mileage reimbursement is provided.
– This role begins as Part-Time, with the potential to move to Full-Time.
$70,000 — $90,000/year
