About Synapticure
Synapticure is a patient- and caregiver-founded company that gives people instant access to expert neurologists, advanced treatments and trials, and full-service care coordination—including behavioral health support—in all 50 states through a virtual care platform. Working with providers and health plans, including CMS’s new GUIDE dementia care model, Synapticure is committed to improving the lives of millions of individuals and families affected by neurodegenerative diseases such as Alzheimer’s, Parkinson’s, and ALS.

The Role
The Care Team Operations Associate (CTOA) is essential to supporting Synapticure’s neurodegenerative care community. This role manages key administrative support activities to ensure patients and caregivers get the help and coordination they need. As a representative of Synapticure, you will communicate with patients, caregivers, medical offices, and external healthcare organizations to help strengthen and optimize care pathways. Success in this position requires flexibility, strong communication skills, and a commitment to delivering compassionate, high-quality service that improves patient satisfaction and outcomes.

Job Duties – What You’ll Do
– Patient and Care Coordination: Act as a liaison between patients, caregivers, and healthcare providers by scheduling appointments, coordinating care, and supporting patient advocacy in line with HIPAA requirements.
– Data Management and Documentation: Gather and accurately document patient details, clinical data, and scheduling information while following standard procedures to maintain quality and efficiency.
– Cross-Functional Collaboration: Work with the multidisciplinary Care Navigation Team and other Synapticure staff to provide a smooth, supportive experience for patients.
– Effective Communication: Represent Synapticure professionally in both verbal and written communication, offering clear, empathetic support and information.
– Administrative Support: Coordinate referrals, manage member mailings, handle correspondence, and provide logistical support to facilitate patient care.
– Problem-Solving and Flexibility: Adjust to changing priorities and use creative approaches to address healthcare system challenges that affect patient care.
– Patient Relationship Building: Quickly build trust with members through nonjudgmental, empathetic interactions that enhance the member experience.

Requirements – What We’re Looking For
– Qualifications: A high school diploma or equivalent is required.
– Experience: Experience working with medical records, communicating with patients, and operating in fast-changing environments such as startups is preferred.
– Technical Skills: Familiarity with Electronic Health Record (EHR) systems and patient scheduling platforms.
– Communication: Strong verbal and written communication skills, with the ability to explain information clearly to patients and colleagues.
– Interpersonal Skills: Demonstrated empathy, organization, and adaptability, with a commitment to patient-centered service.
– Initiative: Self-driven with the ability to prioritize and complete tasks efficiently while adapting to evolving patient and organizational needs.

Our Culture
Synapticure was founded by a patient and caregiver, and we are remote-first. Our values guide everything we do. Even though we work across the country, these principles unite us:

– A relentless focus on patients and caregivers: We deliver an exceptional experience for every patient we serve and put them first in all we do.
– The humanity of neurodegenerative disease: Inspired by our founders and the experiences of families and individuals, we understand the seriousness of our patients’ situations and meet it every day with empathy, compassion, kindness, joy, and—most importantly—hope.
– Curiosity and understanding: We begin by listening—to one another, our partners, our patients, and their caregivers. We communicate with honesty and humility, recognizing that we always have something to learn.
– Embrace the mission: We’re energized by the importance of our work and we act with bias toward progress.
