As an Advanced Practice Clinician (APC) at knownwell, you will collaborate with dedicated physicians, APCs, registered dietitians, nurses, and pharmacists to deliver comprehensive medical care, with a particular emphasis on diagnosing, treating, and preventing obesity and related conditions within a primary care environment. This role provides the opportunity to make a meaningful difference in patients’ lives by helping them improve their overall health.
Schedule: Monday through Friday with flexible hours, requiring a minimum of 32 patient‑facing hours plus 8 administrative hours. Hybrid care model. No scheduled evening or weekend duties aside from phone-based on‑call coverage.
EMR: Athena
On‑Call: Phone-only rotation every 7–8 weeks for one week at a time. Current average call volume is fewer than one call per week (as of 8.1.24). Physician support is available around the clock.
Requirements:
• Master’s degree (MSN) and certification as a family nurse practitioner (ANCC or AANPCB), or a master’s degree in physician assistant studies with NCCPA certification.
• Active DEA license and prescribing authority per state regulations.
• Minimum of 2 years’ clinical experience in ambulatory, family, or internal medicine; experience with chronic disease and weight management preferred. If lacking formal training in weight management, willingness to participate in knownwell training or pursue OMA APC certification.
• Active, unrestricted Massachusetts license or willingness to obtain; openness to additional licensure as needed.
• Strong communication and interpersonal abilities.
• Commitment to ongoing learning in primary care and obesity medicine.

• Commitment to shared decision‑making and supporting patients’ personal health goals.
• High emotional intelligence and the ability to create a comfortable, nonjudgmental patient experience.

• Authorization to work in the United States; visa sponsorship is not available.
Responsibilities:
• Deliver excellent clinical care within knownwell.
• Provide primary and metabolic health care for adults, sharing a patient panel with physicians and other APCs.
• Uphold knownwell’s mission and values of inclusivity in every aspect of patient care.
• Coordinate with the care team to ensure patients receive comprehensive support.
• Complete documentation required for accurate billing, including reviewing remote monitoring data.
• Respond promptly to patient messages routed for clinician attention.
• Care for adults ages 16+ in both metabolic health and primary care.
• Maintain an open panel for all patients, regardless of health status or comorbidities.
• Listen closely to patients’ health goals and tailor care to their individual needs, fostering a judgment‑free environment.
• Provide meaningful encouragement as patients work toward their goals.
• Collaborate effectively with the broader care team.
• Report concerns to site leadership as needed.
• Offer clinical guidance to medical assistants and nursing staff when assigned.
• Stay up to date with all required licensure and certifications.
