This high-value care coordination service is available to all members enrolled in All Included Care (AIC), Virtual Primary Care (VPC), and the Connected Care Program (CCP). We are looking for someone who is committed to delivering world-class service to every member. As a Connected Care Assistant, you will work alongside the clinical team to help members navigate complex or chronic health conditions. You will coordinate care services, provide support, and deliver additional assistance as directed by the clinical team. The ideal candidate is compassionate, knowledgeable about healthcare, and an excellent communicator. CCAs are not permitted to provide clinical advice, make diagnoses, conduct evaluations, or interpret clinical information for members.
Responsibilities include:
• Engaging members through scheduled shifts involving calls, chats, and internal referrals into CCP
• Conducting targeted outreach to connect at-risk members with CCP
• Using autodialer tools for outreach to eligible members
• Managing and triaging care plan task queues
• Coordinating care for members and their families as directed by the clinical team
• Guiding members through available employer-sponsored benefits
• Initiating cases such as Provider Match, Concierge Referrals, Expert Medical Opinions, and Claims
• Completing administrative tasks, including writing letters, obtaining medical records, managing callbacks and referrals, monitoring email communications, and managing platforms like Ovia and ACM insurance feeds
• Attending team huddles when needed
• Participating in ongoing training to stay up to date with healthcare developments
• Following organizational policies and healthcare regulations
• Performing other assigned duties
Requirements include:
• Previous experience in case management or healthcare is preferred
• Skill in motivational interviewing
• Ability to multitask and adapt in a rapidly changing environment
• Strong phone communication and the ability to anticipate needs during calls
• Excellent written and verbal communication
• Strong time management skills and ability to work independently
• High level of organization and ability to meet deadlines
• Proficiency with computer technology and ability to learn new systems quickly
• Experience with health IT systems is a plus
• Familiarity with community healthcare and social services resources is preferred
• Positive and respectful communication with peers, team members, and patients
• Strict adherence to confidentiality and HIPAA requirements
• Availability to work 8–5 PM (PST/CT/EST), Monday through Friday
• Temp-to-perm position
Compensation for this full-time role ranges from $20.72 to $29 per hour, plus equity and benefits. The starting salary will depend on various job-related factors such as education, experience, training, certifications, location, and internal equity.
