About the Role
The Care Navigator provides direct support to individuals impacted by neurodegenerative conditions. This role manages patient relationships, enrollment, documentation, and overall care experience within the Synapticure program, while also helping to develop workflows and processes for the Care Coordination team. Ideal candidates are passionate about elevating patient voices, enjoy supporting vulnerable populations, and thrive in patient-centered work. Successful Care Navigators are thoughtful, organized, curious, compassionate, and empathetic. Job Duties
• Build and maintain supportive relationships with patients and caregivers and serve as their primary point of contact
• Follow HIPAA standards while carrying out care plans and providing ongoing support, helping patients connect with neurology specialists, genetic experts, and other referral resources
• Offer basic health education on diagnoses, treatment and care options, and research opportunities
• Uphold high standards of privacy, confidentiality, and quality in all interactions
• Follow and help improve standardized procedures, documentation practices, and quality controls
• Collaborate with the Care Coordination team and broader Synapticure staff to ensure a seamless, positive patient experience, and support internal communication with external partners
• Develop and sustain productive relationships with medical and community organizations
• Stay informed on research, trends, and advancements in neurodegenerative disease care and represent the program and company confidently to internal and external audiences

Requirements
• Bachelor’s degree in a relevant field
• At least 2 years of experience in direct case or care management in a healthcare setting
• Comfort using technology for remote support, including calls, texts, and virtual visit platforms
• Strong communication, organization, and interpersonal skills for working in a diverse team
• Ability to use scheduling systems for accurate appointment management
• Ability to use electronic health records or care platforms for accurate documentation
• Skilled in collecting and documenting clinical and demographic information promptly
• Strong problem-solving abilities and good judgment on when to collaborate
• Creative in navigating challenges that limit patient care within the healthcare system
• Growth mindset and willingness to go beyond traditional responsibilities
• Adaptability in a fast-paced, frequently changing environment
• Ability to build cooperative relationships with patients, teammates, and community providers

Compensation: $27–$33 per hour

Preferred Qualifications
• Experience supporting patients with neurodegenerative diseases
• Bilingual in Spanish, with strong verbal and written proficiency

Travel Requirements
This is a fully remote role with provided at-home technology. Occasional travel to Chicago headquarters or other locations may be required.

Salary and Benefits
This is a full-time, exempt position with competitive pay and a full benefits package, including health insurance. The salary range varies based on experience.
