The Nutrition Educator provides telephone nutrition counseling to members/patients with chronic conditions and helps them select, prepare, and consume foods that best meet their needs while participating in a meal program for long-term/chronic care. Chronic conditions may include, among others, diabetes, heart disease, and kidney disease. Using a member/patient-focused approach, the Nutrition Educator works with each member/patient to identify and address nutrition-related concerns and topics that are most relevant to them. In addition, the educator connects members/patients with other nutrition and care resources available through the health plan, the healthcare system, or their community.
**Hourly Wage: $26.60–$35.00**
**Position duties may include, but are not limited to:**
– Provide telephone nutrition counseling to each member/patient; the number of sessions varies depending on the program they are enrolled in
– Document educational goals and the care/case plan in the platform
– Schedule call-back/follow-up appointments with each member/patient
– Create nutrition education materials and tools as needed
– Identify community resources for clients as needed
– Stay current in the field by participating in continuing education, reading professional publications, maintaining personal professional networks, and engaging with professional organizations
– Create social media content and videos for articles, blogs, and other posts for the company newsletter and website as needed or upon request
**Required Skills and Experience:**
– Bachelor of Science in Nutrition Science or Dietetics
– **Registered Dietitian certification through the Commission on Dietetic Registration**
– Hold approval/licensure or be able to obtain a license in the state of residence (if required)
– **2+ years of experience providing nutrition counseling/education**
– Strong verbal and written communication skills
– Strong organizational and interpersonal skills
– Regular use of common office equipment, including laptops, computers, and smartphones
– Ability to work independently and take ownership of tasks with minimal daily supervision
– Ability to conduct clinical assessments to determine the appropriateness of nutrition education for each member/patient
**Preferred Skills and Experience:**
– Prior experience in telephone nutrition counseling/education
– Training in health coaching, motivational interviewing, and intuitive/mindful eating
– **RD license in NY, WI and/or OR**
**Physical Requirements:**
– Repetitive hand and finger movements, including wrists, hands, and/or fingers
– Sitting/primarily sedentary work, especially with extended periods of sitting and limited mobility
– Visual ability to perform the role, including peripheral vision, depth perception, and the ability to adjust focus
