The Population Health Manager is a collaborative and strategic clinical leader who supports Aledade’s value-based care goals by promoting health equity and using data-driven insights. This position concentrates on high-impact practices that are underperforming and have significant potential to improve community health by addressing equity challenges. The role leads major change management efforts, performance improvement initiatives, and the rollout of specialized programs, including those focused on Social Determinants of Health (SDOH).
This Manager is an innovative self-starter who develops new population health strategies and builds external partnerships to connect patients with needed resources. The role requires earning trust from multiple stakeholders to successfully drive strategic plans that enhance patient outcomes.
This position involves traveling to partner practices about 50% of the time to conduct detailed assessments and support change.
Primary Duties:
• Change Management and Strategic Execution:
• Create and implement strategic change management plans for complex population health initiatives.
• Work with practice leaders to identify performance challenges and develop tailored, innovative solutions.
• Lead the Gemba Walk process, performing thorough on-site evaluations and creating action plans to support practice transformation.
• Act as the primary expert for launching new population health programs across practices.
• Performance Improvement and Data Utilization:
• Analyze complex performance data to identify trends and areas for improvement in quality, engagement, and cost.
• Research public health data and community-specific obstacles that affect value-based care performance.
• Convert high-level analytics into clear, actionable recommendations for practice teams.
• Oversee the implementation of specialized SDOH solutions to address health equity at the point of care.
• Partner with local hospitals and health plans to obtain key resources for patients with limited access.
• Relationship Management and Collaboration:
• Build and sustain strategic relationships with internal teams and practice leaders to ensure smooth adoption of population health initiatives.
• Develop and deliver advanced training for practice staff on strategic workflows and the use of Aledade’s tools.
Minimum Qualifications:
• Bachelor’s degree in a relevant field
• Active clinical license: RN, Pharmacist, APRN, or PA
• At least 5 years of healthcare experience with a strong background in practice management, quality improvement, or population health
• Demonstrated experience in change management and strategic program implementation; certification in PROSCI, Lean Six Sigma, or similar
Preferred Qualifications:
• Master’s degree in Public Health, Healthcare Administration, or related field
• Strong data analysis skills with the ability to turn analytics into actionable insights
• Extensive knowledge of value-based care, health equity, and SDOH
• Exceptional communication and relationship-building abilities with influence across leadership teams
• Strong interpersonal, communication, and presentation skills
• Familiarity with value-based care models such as MSSP or Medicare Advantage
• Experience using EHRs and population health tools
• Highly adaptable, results-driven, and capable of managing multiple projects independently
• Strong judgment and ability to maintain confidentiality
Physical Requirements:
• Long periods of sitting, extensive computer use, and occasional walking or lifting
• Ability to drive to partner practices; valid driver’s license required
• Ability to travel more than 50% of the time
