Job Summary:
We are seeking an Infertility/Maternity Nurse Care Manager to join our Care and Case Management team. This role requires a CCM certification and an active California nursing license. Candidates must have experience in case and disease management within a health plan, health navigator, or TPA setting. A passion for holistic, patient-focused care is essential to guide members through their healthcare journeys and ensure they receive leading‑edge support and interventions.

As a telephonic Nurse Care Manager, you will assist members through both routine and complex infertility and maternity situations. Reporting to the Manager of Care and Case Management, you will collaborate with a multidisciplinary clinical team—providers, care coordinators, and other support staff—to offer integrated remote care and case management. You should enjoy engaging with members by phone, understanding their needs, answering questions, and advocating on their behalf. You will develop tailored care plans and use your clinical expertise to support members through infertility and maternity care while helping them navigate benefits and resources.

With your CCM certification, you will be equipped to manage complex healthcare needs and ensure each member receives high‑quality care. Nurse Care Managers educate, advocate, and guide members through fertility and family‑building, as well as prenatal and postpartum care, promoting positive health outcomes for members and their families.

Responsibilities
• Provide coordinated, patient-centered virtual care management via phone or messaging to improve health outcomes.
• Develop impactful care plans with members and the care team, helping members reach their goals.
• Assist members in navigating complex conditions, treatments, benefits, and the broader healthcare system.
• Collaborate with members’ local providers to ensure seamless, coordinated care.
• Offer compassionate, ongoing follow‑up and build supportive, trusting relationships.

Qualifications and Requirements
• Bachelor of Science in Nursing (BSN).
• Residence in a compact NLC state.
• Active Compact RN license in good standing.
• Active California nursing license.
• Willingness to obtain additional state licenses.
• Current CCM certification.
• At least 3 years of nursing experience.
• Minimum 2 years in care, case, and disease management.
• Minimum 2 years of labor and delivery experience.
• Minimum 2 years of infertility experience.
• Comfort discussing a wide range of medical conditions.
• Spanish proficiency preferred.
• Strong technical skills, including EMR and digital tools.
• High empathy and commitment to patient-centered care.
• Ability to meet volume goals while maintaining quality.
• Flexibility and comfort with change.
• Strict compliance with HIPAA and security standards.
• Collaborative, responsive, and eager to learn.
• Ability to work across multiple computer and medical record systems.
• Strong teamwork skills with clinical and non‑clinical colleagues.
• Awareness of cultural and socioeconomic barriers affecting members.
• Ability to translate medical information into clear, easy‑to‑understand language.
• Consistent adherence to HIPAA and security guidelines.

Physical/Cognitive Requirements
• Consistent, reliable attendance.
• Ability to remain seated for extended periods.
• Manual dexterity for computer and office equipment use.
• Occasional light lifting up to 20 lbs (such as a laptop).
• Ability to work professionally with leadership, staff, and members.

Pay and Benefits
The U.S. base salary range for this full‑time role varies by geographic zone:
Zone A: $66,850–$93,590 plus equity and benefits
Zone B: $76,878–$99,940 plus equity and benefits
Zone C: $83,563–$108,630 plus equity and benefits
Zone D: $86,905–$112,980 plus equity and benefits

Actual compensation depends on factors such as education, skills, experience, certifications, organizational needs, internal equity, and market conditions. Each zone’s pay structure aligns with local market data. Your recruiter can confirm your compensation zone.
