The Registered Dietitian provides phone-based nutrition counseling for members and patients with chronic conditions, helping them choose, prepare, and eat the foods that best meet their needs while participating in a meal program for long-term/chronic care. Chronic conditions may include diabetes, heart disease, and kidney disease. Using a member/patient-centered approach, the dietitian works with each individual to identify and address nutrition-related concerns and topics that are most relevant to them. In addition, the dietitian connects members/patients with other nutrition and care resources available through the health plan, the health system, or their community.
**Hourly Rate: $26.60–$35.00**
**Job duties may include:**
– Provide phone-based nutrition counseling to each member/patient; the number of sessions varies depending on the program the participant is enrolled in
– Document educational goals and the care/case plan in the platform
– Schedule follow-up call appointments and care follow-ups with each member/patient
– Create educational materials and nutrition tools as needed
– Identify community resources for clients as needed
– Stay current in the field by attending continuing education, reading professional publications, maintaining personal professional networks, and participating in professional organizations
– Create social media content and videos for articles, blogs, and other posts for the company newsletter and website as needed or upon request
**Required Knowledge and Experience:**
– Bachelor of Science in Nutrition Science or Dietetics
– **Certification as a Registered Dietitian (registered as a dietitian) through the Commission on Dietetic Registration**
– Recognition/authorization and ability to obtain a license in the state of residence (if required)
– **2+ years of experience providing nutrition counseling/training**
– Strong verbal and written communication skills
– Strong organizational and interpersonal skills
– Regular use of common office equipment, including laptops, computers, and smartphones
– Ability to work independently and manage tasks with minimal daily supervision
– Ability to perform clinical assessments to determine the appropriateness of nutrition education for each member/patient
**Preferred Knowledge and Experience:**
– Prior experience providing phone-based nutrition counseling/training
– Training in health coaching, motivational interviewing, and intuitive/mindful eating
– **RD licensure in NY, WI and/or OR**
**Physical Requirements:**
– Frequent movement of hands and fingers, including wrists, hands, and/or fingers
– Sitting/primarily sedentary work, especially during longer periods of sitting with limited mobility
– Visual ability required to perform the role, including peripheral vision, depth perception, and the ability to adjust focus
