The Role

A passionate professional who is committed to building accountable partnerships with our members by leading a team of dedicated Health Coaches, Community Health Workers supporting Circulo’s Health Huddle locations and where needed to support other related services as they apply the whole-person approach to health and healing. The manager will oversee the day to day services through authentic communication, trusted interactions and support for the team while the team focuses on helping members with various chronic conditions achieve optimal health.

Who we are looking for

Licensed Practical Nurse or Licensed Clinical Social Worker
3 or more years of experience leading integrative team approach
3 or more years experience educating members in healthy lifestyle, nutrition, movement, sleep/rest ect.
3 to 5 years Health Coach experience helping members manage their chronic conditions and achieving health goals
Strong motivational interview skills

What you will do

Lead the team of health coaches as they guide members to achieve their goals of optimum health
Ensure there is real connection with the work being done at the primary health locations
Develop workflow for daily function of health coach using the pillars of health coaching
Provide guidance to health coaches help members understand the impact of nutrition, physical fitness, mental and spiritual well being
Promote a lifestyle that helps members get results and stay healthy
